The form has items such as oral condition, plaque adhesion status, instruction content, implementation time, and signature of the hygienist in charge, but the fields other than the instruction content and implementation time are blank, and there is no brushing instruction as written. There were no complaints from the doctor or hygienist. However, my medical statement included 80 points for Dental Hygiene Practical Guidance 1, and I paid at the time of payment, but I wonder if I'm the only one. In addition, it took nearly a year to treat one cavity, but three months later, the root of the tooth had accumulated so much pus that the tooth had to be extracted at another dental clinic.
